Required documents
Please click here for my document production requirements in PDF format.
When to send documents
Please send documents as soon as possible after the case is filed and in no event later than seven days before the first date set for the § 341 meeting.
Preferred delivery method
I use BlueStylus – Trustee Collaborative, a web-based document delivery system. Please sign up for this service here using the same email address used to e-file.
If you need assistance, please call customer service at 1-866-878-7831, “1”. Please do not call my office with questions about how to use BlueStylus – Trustee Collaborative, as my staff will instruct you to call the customer service number above.
Until you have signed up for BlueStylus – Trustee Collaborative, please e-mail all documents to [email protected] with the Debtor's last name and case number in the subject line. Please do not send paper documents.
Adjournment policy
If documents have not been produced at least seven days before the § 341 meeting, or if additional documents are requested at the § 341 meeting, I may adjourn the meeting. If the missing documents are not sent to me via e-mail prior to the adjourned date, then the debtor and counsel must appear at the adjourned meeting. If a document is sent after the first § 341 meeting, please do not use Trustee Collaborative. Instead, e-mail documents to [email protected].
Original signatures at § 341 meeting
I will ask the debtor to confirm his or her original signatures on the following documents, so please bring the originals to the meeting:
- Petition
- Declaration Concerning Debtor's Schedules
- Statement of Financial Affairs
- Form 22A (Means Test Form)
Rescheduling § 341 meetings
If you need to reschedule a § 341 meeting, please contact me as soon as possible to request an adjournment to my next meeting date. If an adjournment is available, you must file a notice of the new meeting date and time and serve it on the creditor mailing matrix.
